There have been some grammatical and word choice edits in this version. In addition, I've added Glossary, About the Author and Table of Contents sections.

Since it is too difficult to maintain two separate formats in working manuscripts (one for eBook and one for Print-on-Demand books) I am experimenting with converting the Printed book format for these eBook revisions. There are some spacing issues between paragraphs. My first attempt left no spacing at all and it was difficult to read. My second attempt has good spacing (could be reduced a bit) but has not indented orphaned lines, like dialog that are separated from any paragraph for readability.
